Supporting Success for Hazel
Meet Hazel, a student with a vibrant personality who loves creativity, especially making bracelets and makeup. Hazel is a student at Bolivia Elementary and was referred to the CIS Action for Success Program at the school during the 2025/2026 school year and worked with Success Coach, Ms. Skye. The first step in working with Hazel was to assess all her needs in order to develop a comprehensive and individualized Student Support Plan to help her attain her goals. It was determined Hazel could benefit from tutoring to bring up her grades and from spending time working on her life skills and social connections with her peers. Through her work with CIS, she participated in tutoring and small-group life skill sessions, helping her build meaningful connections with her peers.
Mid-year, Ms. Skye introduced an attendance goal for Hazel as she was not consistently present at school every day, which can negatively impact academic growth. Ms. Skye set a goal for Hazel to attend school for a full-week with special incentives attached to that goal to provide motivation for her attending school more regularly. Hazel embraced this challenge, consistently checked in with Ms. Skye each day, taking pride in selecting her own rewards. Her dedication truly paid off; during the final quarter of the school year, she missed only one day of school. This had a significant impact on her learning as well as her success in building and maintaining peer relationships. CIS is very proud of Hazel and looks forward to continuing to support her needs in the next school year so she can continue on a positive path of academic and social growth.
